Band 6 Community Diabetes Nurse 

An exciting opportunity has arisen for a Band 6 Community Diabetes Nurse to work with our Integrated Diabetes Nursing service in the borough of Barking and Dagenham and the wider BHR teams.

We are looking for an enthusiastic and motivated individual, with excellent communication skills, who is able to work autonomously, as part of the team and to be an integral member of the wider Diabetes teams in supporting people managing the complexities of diabetes management.

The ability to travel effectively across the boroughs is necessary.

Main duties of the job

This role requires facilitating patient structured education, having a sound knowledge of the principles of diabetes management and care and a keen interest in professional development & advancing with technology. Therefore experience of teaching and good IT skills is essential.

A minimum of 1 year community experience with Patients with Diabetes is essential

The post holder will be responsible for providing; supporting and developing the delivery of high quality; evidence based Diabetes clinical practice throughout the local community, in health centre clinics and patients’ homes. The successful applicant will be required to work closely with the lead Diabetes consultant, GPs, nursing colleagues and other members of the multidisciplinary team.

Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

Probationary Period

This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff). 

High Cost Area Supplement - London

This post attracts payment for High Cost Area Supplement of 15% of the basic salary with a minimum payment of £4,551 and a maximum payment of £5,735 per annum.
