Main duties of the job
The post holder will work as a complex case manager, without direct supervision. You will be involved in delivering integration, working with GP's, Social services and the third sector organisations.
You will be providing case management, facilitating early discharge from hospital and preventing unnecessary/unplanned hospital admissions where appropriate as well as supporting healthcare closer to home.
The service currently operates from - , Monday to post holder will be expected to support their team, department and organisation to achieve the Trust's Values in their day to day post holder will be a Registered Nurse, with at least 2 years community experience of managing a complex caseload of will be responsible for a caseload of patients with complex health and social care needs. You will work autonomously to case manage these clients and use advanced nursing skills, such as assessment and prescribing to manage their conditions in the community.
They will work as part of the integrated care team, working within and across professional and organisational boundaries. Liaising with GPs, other professionals and statutory and voluntary agencies he/she will deliver individualised care to patients.

Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.
High Cost Area Supplement - London
This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,551 and a maximum payment of £5,735 per annum pro rata).

The Community Matron :
Will manage a caseload to maximize efficacy and effectiveness in their role in order to provide best service to the patient. Will utilize a range of quality improvement tools to review professional practice and initiate and evaluate changes to practice when required. Will access Clinical Supervision and professional support from sources agreed with the line manager in order to support critical reflection and support safe practice. Will support and supervise colleagues and provide targeted education, training and development as required. Will engage positively in performance management and participate in the review and development of their Personal Development Plan As required.
Will demonstrate effective clinical and professional leadership in regards to developing the strategy for the development of services surrounding Long Term Conditions
