Job summary

An exciting opportunity has arisen for Senior Community Nurse or District Nurse to join our Integrated Teams in Redbridge. The applicant is required to have a District Nurse Certificate or experience working as a community nurse for at least 2 years and willing to undertake the band 6 community nurses development programme.

Thesuccessful candidateswill be expected to support their team, department and organisation to achieve the Trust's Values in their day to day work. These are the 5P's:

Thesuccessful candidateswill be a Registered Nurse (RN12-NMC) responsible for patients allocated to them and will assess, manage, plan and deliver care, including being professionally and legally accountable.

Thesuccessful candidateswill support their peers and team leader and be an effective team member whilst working on their own initiative. They will supervise pre-registration students. They will have a commitment to collaborative working actively supporting and liaising with other health and social professionals and agencies.

Main duties of the job

The post holders will lead and manage a Community Nursing Team, geographically aligned to GP practices and be responsible for the management of a caseload ensuring the provision of a high standard of nursing care to patients and their families in the community. He/she will work as part of the integrated primary/community health care team, working within and across professional and organisational boundaries. Liaising with GPs, other professionals and statutory and voluntary agencies he/she will deliver individualised care to patients. The Senior Community Nurse works without direct supervision and provides leadership to the community nursing team.

High-Cost Area Supplement

This post also attracts payment for High-Cost Area Supplement of 15% of the basic salary (minimum payment of £4,313 and a maximum payment of £5,436) pro rata for part time staff.

Job responsibilities

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

Qualification

Essential

Current NMC Registration Registered Nurse (RN) Evidence of professional up-dating Agreement to undertake the following modules (band 5/6 Development programme), Academic ability to study at degree level

Desirable

Relevant post registration course and experience Palliative Care, Diabetes and Tissue Viability Mentor Qualification

Experience

Essential

Has experience of team working that will assist the individual to manage an effective team Interagency and collaborative working Evidence of teaching and assessing individuals Providing complex packages of care 2 years community Experience

Desirable

3 years post registration experience Ability to manage staff conflict and difficult situations Experience of ensuring practice is based on sound evidence and clinical audit

Knowledge

Essential

An awareness of NHS Plan, NSF and clinical governance priorities Awareness of the content and application in practice of the NMC Code of Professional Conduct A good range of clinical skills, including assessment Understand the importance of chronic disease management and rehabilitation in the community

Skills

Essential
