About this role… The Thames Reach Outreach teams operateacross London and help those who are rough sleeping to find routesaway from the street. The teams respond rapidly and work flexiblyto reach people. Individuals are assessed to identify their supportneeds and support plans are created, including work to obtainidentification, access health services and refer to otherspecialist agencies, including those offering immigration support.You will: - Lead regular outreach shift with partner agencies orvolunteers, assessing new clients and quickly building trustingrelationships. - Manage a caseload of rough sleepers, identifyingthe quickest and most appropriate route off the street, anddevising a plan to achieve this. - Improve the local response torough sleeping by coordinating joint working and collaborationbetween agencies. - Represent the team at multi-agency meetings andcase conferences. - Identify gaps in local provision and researchnew pathways off the streets for a variety of rough sleepers,including those with no recourse to public funds. - Work across avariety of databases to accurately record your work. To succeed asthe Support Worker you will have: - A strong commitment toproviding a high standard of service - Experience working withpeople who have rough slept or vulnerable adults and supportingthem to access services - Excellent organisation and timemanagement skills in order to remain strongly outcome focused, withthe ability to coordinate a high number of cases and complete taskspromptly and efficiently. - Excellent communication skills andconfidence in public speaking when representing Thames Reachexternally - Experience in forming strong working relationshipswith partner agencies, being able to challenge them when required -Strong IT skills with the ability to use Outlook, online recordingsystems, Excel, and Word, using a tablet and a smart phone.
