Job overview

An exciting opportunity has arisen for a Clinical Nurse Lead within Brentwood & Billericay Community Health Team, this is an exciting and autonomous role supported by the District Nurse Team Leads and Operational Lead.

Working in a clinical role across the borough the successful candidate will review and implement plans of care for complex patients across the borough, acting as a clinical expert and role model to the Community Nursing and Mental Health and Well being teams 

Main duties of the job

The post holder will demonstrate excellent communication and negotiation skills, as well as a sound knowledge base linked to advanced physical assessment and treatment planning of individuals with complex comorbidities that require a multidisciplinary approach from the Primary Care Networks(PCN).

The Clinical Nurse Lead will be required to support the community nurses and the PCN partners to coordinate and deliver the care to this cohort of complex patients, ensuring that services wrap around and support them to maintain their health and wellbeing in their normal place of residence.

The post holder will be a Registered Nurse (RN12-NMC) and will be required to support the Community nursing teams and the Primary Care Networks(PCNs) to coordinate and deliver the care to patients within the respective localities, including being professionally and legally accountable.

Working for our organisation

Starting with NELFTNELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

Probationary Period
This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

High Cost Area Supplement – FringeThis post also attracts payment for High Cost Area Supplement of 5% of the basic salary (minimum payment of £1,258 and a maximum payment of £2,122 per annum).

The post holder will support their peers and Operational Lead and be an effective team member whilst working on their own initiative. They will supervise pre-registration students. They will have a commitment to collaborative working actively supporting and liaising with other health and social professionals and agencies.
