North East London Foundation Trust have an exciting opportunity for a Community Matron Homeless pathway in Waltham Forest, ICT Central, Integrated Community Team.

Main duties of the job

The aims and intended outcomes of the service which are set out below are based on the commitments in the ‘Healthcare and people who are homeless Commissioning Guidance for London’, which was produced by Healthy London Partnership (HLP) revised April 2019

The service provider will carry out a triage and a comprehensive physical health check for all residents that consent to this assessment. Based on the outcome of this health check, a plan will be put in place to address any unmet needs or sign post the resident accordingly. All residents found not to be registered with a GP will be supported in registering with one and supported to access other health services or social care / voluntary sector support

The service currently operates from - , Monday to Friday.

The post holder will be a Registered Nurse, with at least 2 years community experience of managing a complex caseload of patients. You will be responsible for a caseload of patients with complex health and social care needs. You will work autonomously to case manage these clients and use advanced nursing skills, such as assessment and prescribing to manage their conditions in the community.

They will work as part of the integrated care team, working within and across professional and organisational boundaries. Liaising with GPs, other professionals and statutory and voluntary agencies he/she will deliver individualised care to patients.

This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focussing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

High Cost Area Supplement - Outer London

This post also attracts payment for High Cost Area Supplement– 15% of basic salary, with a minimum of £4,551 and maximum of £5,735, pro rata for part time staff

Detailed job description and main responsibilities

Will manage a caseload to maximize efficacy and effectiveness in their role in order to provide best service to the patient.


 Will utilize a range of quality improvement tools to review professional practice and initiate and evaluate changes to practice when required.


Will access Clinical Supervision and professional support from sources agreed with the line manager in order to support critical reflection and support safe practice.


 Will support and supervise colleagues and provide targeted education, training and development as required.

Will engage positively in performance management and participate in the review and development of their Personal Development Plan As required.
 
Will demonstrate effective clinical and professional leadership in regards to developing the strategy for the development of services surrounding Long Term Conditions
