We arelooking to recruit a CHC Nurse Assessor / Case Manager.This is a patient facing role and the post holder will deliver the end to end CHC/ CCC / Individual funding process, supporting the CHC / Complex Case TeamLeaders and the Operational ClinicalLead in all elements of service delivery.

Main duties of the job

Facilitating Continuing Healthcare/Funded Nursing Care assessments andreviews for patients across all aspects of care this includes appeals andretrospective assessments. Thisnecessitates a high level of assessment skills and requiring tact, diplomacyand the ability to liaise effectively with colleagues, health careprofessionals, patients' carers and family members. The post holder willrequire Registered Nurse qualification.

Job responsibilities

Thetimely review of all Continuing Healthcare/Funded Nursing Care patientsfunctioning as a central point for referral, assessment and support. This willcontribute to the maintenance of the appropriate level of funding support, theaccuracy of the Continuing Healthcare client database and the commissioning ofappropriate continuing healthcare funded packages of care to meet theidentified care needs of individual patients deemed eligible for support. Offeringcontinuing support and advice to family and carers in complex situations,necessitating a high level of interpersonal and communication skills; supportingthe local resolution process in the case of complaints/appeals/retrospectivecases and providing information on a timely basis. ProvidingSpecialist Nursing Advice to secondary care, primary care, nursing homes etc tofacilitate the continuing healthcare process. Developing and maintainingeffective and appropriate communication networks with health and social careprofessionals and also act as a resource to them. Supportingcommunity Matrons/District Nurses in their case management roles for complexcases who are eligible for continuing healthcare. Ensuring there is an effective, up to datepatient caseload plan which meets the requirements of the individual CHCservice. Co-ordination of the assessment process andwhere necessary writing key elements of the patient care plan to reflect presentinghealth needs. Working with relevant health professionals,CHC colleagues, business support staff, senior leads and other support staffwithin the CHC team to ensure delivery of key targets and products required. Supporting the publication of regular reportsas may be required on progress within a given caseload of patients/sphere ofservice. Exercising professional judgement as necessarywhen communicating information on behalf of the CHC service and application ofthe associated criteria. Interpreting national and regional guidancerelating to the implementation of the National Framework and subsequentdevelopments within the CHC service. Delivering formal and non formal educationprograms to Community and hospital staff including nursing/residential homes,GPs, community nursing and therapy staff relating to the assessment andmanagement of CHC patient groups in line with the latest research, evidence andDOH/NICE guidance. Ensuring timely "End of life" Careand reviews, following the national guidance for clients Fast Tracked throughthe CHC system. Facilitating smooth patient flow across allaspects of the patients journey including liaison with primary care, secondarycare, community venues and specialist services Support and inform the targeting of resources,monitoring, implementation and evaluation of the tasks/projects by providinghigh quality support including complex information and analysis, communicationsand stakeholder management Ensure accurate and open communication andco-ordination with a range of organisations and individuals, researching anddrafting correspondence and papers and ensuring the management of specifictasks, lead reporting and analysis across a range of specialties, functions andprojects Be a key member of the team as well assupporting effective communication and stakeholder management, both internallyand externally The job description and person specificationare an outline of the tasks, responsibilities and outcomes required of therole. Experience

Essential

Demonstrate relevant post registration experience as a registered nurse with some knowledge of case management or transferrable skills. Experience of working both in a team and on own initiative. Experience of working with the multi-disciplinary team and with a wide range of health and social organisations. Previous experience in similar role in public sector.

Desirable

Experience of conducting CCC assessments and case management experience of children with complex health and behavioural needs. Experience of supporting change management processes, along with evaluating these processes and an understanding of a performance lead service. Further training or experience in project management, financial management or supporting change management processes.

Qualifications

Essential

Registered Nurse (Children, Mental Health, Learning Disability) Educated to degree level in relevant subject or an equivalent level of experience working at a similar level.

Knowledge and Understanding

Essential

Must demonstrate an excellent knowledge and understanding of the care needs presented by individuals with a variety of conditions and disabilities. Must have a working knowledge and experience of current government legislation around safeguarding strategy and the process of implementing this.

Desirable

Knowledge and awareness of resource management, including awareness of Personal Health Budgets (PHBs).

Skills and Competencies

Able to make recommendations on difficult and/or contentious issues where there may be a number of potential courses of action. Able to demonstrate problem solving skills and ability to respond to sudden unexpected demands at short notice. Able to demonstrate experience of line management abilities and provide team/peer supervision as required. Proficient and competent use of Office 365 packages especially Word, Outlook, and at time use of Power Point.

Able to provide specialist training around specific subjects, advice and support on own role/responsibilities where necessary. A knowledge of recruitment processes within NHS and/or relevant health or social care organisations. Exposure to and awareness of change management processes within area of service delivery. Experience of performance and governance strategies and the development and implementation of improvement programmes.
