An exciting opportunity has arisen within the Intensive Home Nursing Team for a Band 6 Nurse delivering high quality patient care within palliative care.

The service which is community based in the Michael Carlisle Unit in the Southwest of Sheffield provides palliative and end of life care (EOLC) for people who are approaching EOL and their preferred place of care is at home.

This post will provide leadership which will help shape the service provision for the future. This role requires close working relationships with patients, families, Continuing HealthCare, Community Nursing, Discharge teams, Hospices and Social Care Providers.

We welcome applications from experienced nurses who have proven leadership and service improvement expertise. Some hands-on clinical care will be required and you must have a valid UK drivers’ licence and have a vehicle for work purposes.

Working Hours/Pattern: 25.5 hours a week for 3 days. Please note that the service is 7 day service, 365 days a year and a working days consists of 8.5hrs.

Main duties of the job

To support the Operational Service Lead and Clinical Services Manager in providing coordinated, high quality effective care to patients who are requiring End of Life care at home.

Deputise in the absence of the Operational Service Lead and provide daily support with the operational management of the Intensive Home Nursing Service.

To provide supervision, mentorship, and professional leadership support for the Nurses, Health Care Support Workers and Administrators.

To ensure that high quality patient focused care is delivered by IHNS and that it meets the expectations of individual patients and reflects strategic direction of the service.

To work flexibly to ensure care is delivered 24 hours a day, 365 days per year, including participating in weekend working.

Education and Qualifications

Essential criteria

RGN Level one NMC registered To have completed the national PEAP programme for practice supervisors or equivalent teaching and assessing qualification for those with ENB998 or equivalent qualification / experience Academic courses relevant to role to complement academic qualification and experience e.g. postgraduate qualification in palliative care. Continuous professional development

Desirable criteria

First degree or evidence of working towards one (demonstrating graduate attributes)

Experience

Essential criteria

Relevant post registration experience at AFC Band 5 across a range of patient conditions Ability to lead, motivate and develop a professional team Experience and knowledge of Palliative Care and End of Life approaches and care Completion of Band 5 clinical competencies

Desirable criteria

Experience of implementing service improvements Relevant experience at junior sister/ charge nurse role

Skills and Knowledge

Essential criteria

Ability to communicate effectively including active listening, writing, and verbally, with individuals on a one-to-one basis and with motivational skills Good understanding of the national strategic context of the NHS Excellent performance management skills.
