Rapid Response Assistant Practitioner / HCA / Hybrid worker 

We are seeking a motivated and dynamicBand 4 HCA or Hybrid worker ,with an interest in general physical health for this exciting opportunity to join the Waltham Forest Rapid Response team

 The Rapid response is a forward-thinking and innovative service that is community based and works in a fully integrated way, with nurses physiotherapists, occupational therapists, social workers community psychiatric nurse and healthcare support all working side by side to ensure an integrated and holistic approach to the care of people accessing our service.

The team is expanding in response to the new national Urgent care standards so this is the perfect time to join a progressive and innovative multidisciplinary team aimed to support the population of Waltham Forest, providing urgent Multidisciplinary (MDT) response within 0-2 hours to prevent avoidable hospital admissions. The team works across 7 days a week, 24 hours a day and requires flexible working across various shifts. 

You will be required to use your own car and work mileage and parking are claimable.

Main duties of the job

You will be expected to carry out clinical visits including falls pick up to patients in their own homes and residential and care homes under the direction of the nurses/therapist delivering a variety of nursing and therapy skills to the patients home environment and work as part of the multidisciplinary team.

The aim is to support and treat suitable patients in the community, with a range of escalated health and social care needs.

 The right candidate will have :

2 years experience either as a rehab assistant/Practitioner or as HCA/Nurse associate Established and competent extended practice at band 3 A desire to learn and support new ways of working As the role spans a wide MDT, remit in-house training will be offered as required for identified skill gaps.

The successful applicant must be flexible, a good team worker, used to working under pressure, have excellent interpersonal and IT skills and be able to communicate effectively with patients, carers and other members of the MDT.

We are interested in hearing from both assistant practitioners and nurse associate’s with proven clinical and communication skills to work as part of our friendly, supportive and innovative team. Previous experience in a community setting in the NHS is highly desirable but not essential.

Working for our organisation

Starting with NELFT 

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

Probationary Period

This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

High-Cost Area Supplement

This post also attracts payment for High-Cost Area Supplement of 15% of the basic salary (minimum payment of £4,313 and a maximum payment of £5,436) (pro rata for part time staff).
