Job summary

The successful candidate will need a minimum of 5 years post registration experience, be able to function as an autonomous practitioner with advanced clinical skills and a good knowledge of long term conditions. It is also important that the candidate can demonstrate flexibility, good communication skills, an ability to work as a team member as well line manage the Assistant Practitioners within the team.

The post holder will work as a case manager and will be involved in delivering integration, working with GPs, Social Services, Care Coordinators and the Community Nursing/Therapy Services. You will be providing case-management, care co-ordination and support, to patients in their own homes to avoid unnecessary hospital admission, reduce hospital stay and maintain health and well-being for the patients. We work closely with our Acute sector going into to the local Hospital to facilitate early discharge.

Main duties of the job

This post requires that you work flexibly across all Localities in Havering within the Integrated Community Team.

This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focussing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex

High Cost Area Supplement - London

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,551 and a maximum payment of £5,735).

Current NMC Registration Registered Nurse (RN) Nurse/Prescriber/Non-Medical Prescriber Evidence of professional up-dating Academic ability to study at degree level
