We are seeking a dedicated and compassionate nurse to join the specialist community stroke nurse team. If you have a passion for stroke care & rehabilitation and want to support stroke survivors to have the best possible quality of life after stroke, then this is your dream job! 

The community stroke service provides specialist assessment, care & rehabilitation. The role is instrumental in guiding patients, relatives and carers through the recovery process.

This is a rare opportunity to work in an established and supportive large multi-disciplinary team covering a diverse population of Sheffield. 

Candidates must have a driving licence and access to a vehicle for work purposes. 

Main duties of the job

To work within a stroke interdisciplinary team to provide high quality specialist nursing care to patients following a stroke in their own home/communities, in order to allow patients to achieve their optimal level of independence and maximum rehabilitation potential.

To work collaboratively across the Integrated Community Stroke Service to ensure a seamless provision of stroke services across the acute hospital, inpatient rehabilitation and community teams.

To work autonomously, carrying out the assessment, implementation and evaluation of evidence based specialist treatment programs for patients following a stroke, who may have a range of impairments and complex health needs.

To provide specialist advice and education to patients, families and carers to ensure those affected by stroke have the appropriate support for recovery and life after stroke.

To supervise and support junior staff and students.

Contribute to service development and data collection within the Integrated Stroke Services.

The role will involve travelling across the city as well as utilising technology to offer services virtually.

This vacancy is advertised on a fixed term contract basis for a period of 12 months due to the post being part of an organisational change within the Integrated Stroke & Geriatric department. The fixed term contract status of the post holder will be reviewed in accordance with the Trust's legal obligations regarding fixed term contracts and the management of organisational change.

Please also note any secondment must first be agreed with your current line manager.

Qualifications

Registered Nurse Qualification (1st Level)

Desirable criteria

Non-medical prescribing or a willingness to work towards. Relevant Stroke specific training

Knowledge, Skills and Attributes

Essential criteria

Up to date knowledge of stroke specific nursing assessments and treatment Knowledge of long-term conditions management (LTC) including pressure care, continence care, medication assessment, end of life care Evidence of achievement of appropriate competencies for clinical grade Current knowledge of Local, Regional and National policies and Drivers. Evidence of continuous professional development Effective communication skills, verbal and non-verbal Motivated and resourceful Able to record accurately, clearly and concisely Ability to work as an autonomous practitioner Evidence of working with external agencies and networking skills IT skills Good prioritisation and time management skills. Ability to show flexibility to respond to the needs of the service Able to work under pressure Ability to work in a variety of settings

Desirable criteria

Extended knowledge of stroke specific assessment and management Presentation skills Knowledge or experience of audit, evaluation and research Advanced clinical reasoning skills.
