We provide an Emotional Wellbeing Mental Health Service for children and young people across the whole of Essex. We are the provider of all age eating disorder services and child and adolescent mental health services across Kent and Medway.

With an annual budget of £355 million, we provide care and treatment for a population of circa million. We employ approximately 6,000 staff who work across 210 bases in London, Essex, Kent and Medway.

An exciting role for an Advanced Clinical Practitioner has been created to join the dynamic CTT team that covers Barking & Dagenham, Havering, & Redbridge. Community Treatment Team (CTT) aim to avoid unnecessary admissions to the acute hospital settings by managing patients in crisis in their own homes. The post holder will deliver high quality holistic nursing as part of a multidisciplinary team within the accident and emergency and community setting, working with patients towards rehabilitation, self-management and independence throughout the continuum of care.

Main duties of the job

Job Purpose:

The post holder will use advance assessment and diagnostic skills to assess and treat patients. The post holder will be an autonomous practitioner, collaborating with colleagues in primary care, community and hospital settings as appropriate.

The post holder will:

Be a Qualified Advanced Nurse Practitioner Act as a role model Be responsible for undertaking comprehensive health assessments, physical examination and ordering tests for patients in Rapid Response Use advanced clinical decision making and assessment skills, coordinate care planning and initiate referrals to diagnostics for appropriate tests for this population. Be involved in establishing delivering and leading a comprehensive service within community services You will play a leadership role within the team and work with external stakeholders such as London Ambulance Service and Social Services to develop new ways of working and through networking increase the number of referrals from the acute and community. Probationary Period

This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process, new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

High-Cost Area Supplement - London

This post also attracts payment for High-Cost Area Supplement of 15% of the basic salary (minimum payment of £4,551 and a maximum payment of £5,735).

Detailed job description and main responsibilities

Key Responsibilities:

Clinical Practice:

Be professionally and legally accountable for all work undertaken and to practices at an advanced level of professional autonomy and accountability that is within national guidelines and Nursing and Midwifery Council’s Code of Conduct. Provide clinical leadership and maintain credibility through practice and professional update. Act as a role model Practise evidence based nursing care to ensure high quality care. Act as an expert nurse and resource for patients, families, and staff. Admit, discharge or transfer patients to and from the service as their needs require. Undertake advance clinical assessments of patients referred to the service to enable prompt assessment, diagnosis and treatment according to agreed policies, protocols and guidelines. Clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly. Order and interpret blood tests and initiate appropriate action as required. Be competent in using a range of diagnostic tools including bladder scanner, point of care testing & ECG Provide and request appropriate diagnostic and/or therapeutic interventions in order to provide a holistic assessment of the client group. Maintain and develop own repertoire of advanced clinical assessment skills including physical examination, assessment of continence, mental health screening, nutritional status, falls assessment. Prescribe medication within own level of knowledge and competence and within the scope of the Trust’s Policy for Non-Medical Prescribing qualification. Facilitate the further development of clinical skills for all nurses caring for patients. Ensure self and staff are provided with adequate protection if exposed to body fluids, odours and infections. Be aware of needs and ensure that staff, patients and relatives use personal protection equipment provided and adhere to Trust policies in relation to health and safety and infection, prevention and control. Support staff as required in the management of challenging situations. Complete documentation relating to patient care and ensure that staff maintain and update documentation as changes occur according to Trust policy. Encouraging nursing staff through role modelling to apply critical thinking through the process of clinical assessment of patients or problem solving. Follow this process through the planning and evaluation stages of patient care and to effectively document this process in the patient records. Liaise and work closely with relevant multi-disciplinary team members, including General Practitioners and therapists in the provision of professional service. Provide support for anxious patients and/or relatives demonstrating empathy and understanding in a professional manner. Provide empathetic support when involved in the process of giving patients and/or relatives bad news; for example, informing patients/relatives of changes of condition. Prioritise, organise and manage own workload in a manner that maintains and promotes quality. Assist in further development of the service and wider organisational strategies. Take responsibility for own learning and performance including participating in clinical supervision and acting as a role model, as well as continuous preparation for annual personal appraisal and development review. Contribute to the development of local guidelines, protocols and standards.
